CASE 1: Lucindais sufferingfrom depression and diabetes and need help from a team of providers ranging from a mental health therapist, primary care physician and a nutritionist or dietician in other to reduce the symptoms of depressions and also manage and control her diabetes issues. With all Lucinda’s health issues, a mental health provider will help Lucinda to express her emotions and also help her with her mood in other to communicate her feelings well. A primary care physician will also help her with medications that will control her blood sugar and reduce the symptoms of diabetes and nutritionist or a dietician will also help her to choose healthy foods and also have meal plan that are healthy for diabetic patients (nimh.gov 2020).
The appropriate treatment goals and objectives for Lucinda will be for her to first engage in daily physical activities thirty to sixty minutes a day for five days, eating only healthy foods, avoid foods high in sugar and carbohydrate, consuming only low or no sodium diets and drinking plenty water. Again, she must visit her primary care physician on a regular basis and strict adherence to taking all medications prescribed by the physician. The objective for this goal help Lucinda to have a coping strategy deal with a depression symptom whiles controlling and managing the issues of diabetes (Kathol, et al, 2010).
To keep Lucinda safe, she must first be in a healthy environment for instance a home with all the basic amenities like water, power air/heat etc. She must also be in a place that’s is peaceful and quite so she can concentrate on only positive thoughts that promotes healthy mental health state. The plan may also contain ways for her to identify her needs, and how to honor them This plan will help feel safe and heal comfortably in a safer and peaceful environment (Kathol, et al, 2010).
CASE 2: Paul is dealing with congenital heart disease, family anxiety and school nonattendance. Paul’s illness seems related to each other so he will need team providers to collaborate inn treating him. This collaboration of team doctors will help to improve the quality of treatment that he will be receiving from the integrated health care setting. The team of providers will be a primary care physician, a behavioral therapist and her psychologist to help with Paul in his progression (Kathol, et al, 2010)
The appropriate treatment goal for him will be healthy eating and regular exercising to help with regulate his heart rates. I will also set a family therapy session for all the family members and also individual counseling to know and understand their issues. This session will give each individual the chance to express their emotions and also understand and respect each other’sfeelings. Will also use CBT principles to help Paul to differentiate between healthy and unhealthy behaviors. And also encourage and motivate him to improve on his school attendance. The objective for this goal is for me and Paul to develop and recognize activities and behaviors that promotes and cause Paul’s problems so we can address it and to discover healthy coping strategies to help him stay healthy and manage his anxiety level. This treat goal will also help Paul with problem solving strategies (mayo clinic, 2020)
In addition to treatment, Paul still need to be in a safer environment in other to achieve his goals and get treated as well. Therefore, I will make sure he has a good accommodation where all his necessary basic needs are available, for example healthy foods, water, light air/heat and access to transportation. I will also make sure he is in a peaceful environment that will give him the comfort and away from negative influences that may affect his treatment goals.
It is not easy for individuals who are struggle with mental and physical health to cope without seeking for help. However, if the individual seek help form the right providers, they become less stressful and are able to learn ways and strategies to manage their health issues in other to have a healthy mind and body.
