Read the case study of “Jack” located in the Topic 4 materials and provide the appropriate DSM-5 diagnoses in descending order, from the dominant, to the least dominant. For each diagnosis you assign, provide an explanation of the diagnostic criteria you assessed to be compelling, as found in the DSM-5 diagnostic criteria monograph for each disorder.

ANS:

(I) Bipolar I disorder

Bipolar I disorder is caused by different conditions and exhibit several symptoms so can be hard to diagnose. However, Jack seems to be suffering from bipolar I disorder from the below symptoms which are most evident. To begin with, Jack has the tendency of becoming easily distracted and therefore forgets easily. There reached a point whereby he has to have a notebook where he needs to note down what he has to do for him not to forget. Another symptom exhibited is that Jack`s moods became grumpy (American Psychiatric Association, 2013 n.p,). In the past, Jack was a happy and energetic individual, but the characteristics have changed to becoming dull. Most of the times he is depressed and therefore a sign something is disturbing him mentally. Jack has decreased the need for sleep to an extent that he had to be prescribed a sleeping aid to help him sleep and relax. These are signs that show Jack is likely suffering from Bipolar I disorder.

(II) Manic Episode

Jack exhibits a funny way of communicating like the instance when his wife had to answer most of the questions he was asked. The context describes that Jack`s responses were short-closed ended responses and just shrugs. The responses, however, were followed by charming smiles which showed dramatic mannerism. It was unlike him in the past since he was talkative and easily interacted with people. Attention being drawn away is another symptom which would show that he is suffering from a manic episode.

(III) Cyclothymic disorder

Lastly, Jack seems also to be suffering from cyclothymic disorder though its evidence is faint. Jack has emotional ups and downs. He exhibited periods of low moods whereby he showed less empathy towards Jill. He also showed signs of increased frustrations with himself and those they lived with. These symptoms show that his moods were low and he was depressed as he never had hope for being healthy again.
